TERMS AND CONDITIONS
1. Purpose and Scope
The purpose of nutritional counseling is to enhance overall health, vitality, and well-being through education, dietary guidance, and the use of non-medicinal nutritional supplements. Julie Six, Functional Diagnostic Nutrition Practitioner (FDN-P), does not diagnose, treat, or cure any disease, disorder, or medical condition. Julie Six is not a licensed Dietitian, Naturopathic Doctor, or Medical Physician.
2. Medical Disclaimer
If Julie Six, FDN-P suspects the existence of disease, disorder or condition, the client will be informed of this suspicion. However, the client acknowledges this is not a diagnosis or conclusion about the state of the client's health and that the client will be informed and advised to seek medical attention from a licensed healthcare provider. Nutritional counseling is not a substitute for medical care.
The client assumes full responsibility for consulting a physician regarding any suspected health concerns.
Liability Waiver: Julie Six, New Leaf Nutrition LLC, and New Leaf Holistic Health are not liable for any health outcomes resulting from nutritional counseling or services provided.
3. Payment, Cancellations, and Refunds
All services must be paid in full before scheduling.
Holistic Health Rebuilding Programs 3 months or longer may be eligible for a payment plan under contract.
A minimum of one business day's notice is required for appointment cancellations or rescheduling.
Cancellations or rescheduling requests made with less than 24 hours' notice will be charged 50% of the appointment fee to compensate for lost time.
Refunds: Unused sessions may be eligible for a refund or can be held as a retainer to be used in the future. If fee increases have occurred, the client will need to pay the difference prior to scheduling. Contact Julie Six, New Leaf Holistic Health to discuss options.
4. Client Responsibility & Practitioner’s Right to Refer
All recommendations are voluntary, and clients participate in nutritional counseling of their own free will.
Clients are fully responsible for their health decisions.
Nutritional counseling should be considered complementary to other healthcare services, not a replacement.
Right to Refer: Julie Six reserves the right to decline cases outside her professional scope and will refer clients to an appropriate provider if necessary.
